This is the spreadsheet that Team managers will use to collect the uniform orders from the individual players and then compile those orders on to one team order form for the Club Uniform coordinator. This form is used to record and submit volunteer hours completed by scholarship recipients as required by Gilbert Soccer Club. Monthly submission of the report is mandatory even if no volunteer hours were completed. Please see the Scholarship Application Form if additional information is required.
